After inserting a Dolphin Pen into any USB port a message 'You need to format the disk in drive x: before you can use it. Do you want to format it?' appears

The above message is shown every time you plug in a Dolphin Pen, and the software fails to load

This may occur if the pen hardware has become corrupt. Please contact Dolphin Product Support to arrange for your pen to be returned to Dolphin for repair. If you did not purchase your pen directly from Dolphin, please contact your local dealer.

Product Support will probably ask you to return the pen Recorded Delivery for inspection.
